ADR-0005: Agent Adapter is separate from Execution Environment

Context
Coupling provider and runtime creates a cross-product of Claude×worktree, Claude×Docker, Codex×worktree, etc.
Decision
Agent Adapter/Strategy owns provider protocol. ExecutionEnvironment owns where work runs.
Consequences
First environment is LocalWorktree. Additional runtimes can be introduced without rewriting provider integrations.
Change rule
If implementation evidence proves this wrong, update the canonical architecture/product document first and then supersede/amend this ADR. The ADR is not allowed to keep current docs stale.
