ADR-0018: Structured escalation uses a Forge-controlled tool

Context

The supervisor cannot infer choices the model considered but never mentioned. Mature harnesses replace/native-adapt question tools for reliable UI.

Decision

Escalation-dependent modes rely on a Forge-injected/controlled tool or equivalent structured backchannel, not stdout inference.

Consequences

Adapter preflight verifies the tool is actually callable. Failure disables dependent modes rather than silently degrading.

Change rule

If implementation evidence proves this wrong, update the canonical architecture/product document first and then supersede/amend this ADR. The ADR is not allowed to keep current docs stale.
